Thanks for this. To me there just seems something inherently not right about discussing something so private in public. Even if the records could be tracked and audited and all the other issues you raise addressed, I would still not feel comfortable. If I was in a supermarket and a patient approached me about their medical condition I would not feel happy discussing their problems there either.

I think if we're asking the question in the first place (am I being too paternalistic?), that it's worth discovering if we are.

Before, we used to say, "e-mail is not a way to communicate with patients," today we say, "Twitter is not a way to communicate with patients." Is there a therapeuticly advantageous way that we might partner with a patient using these media?

Ok, Ted. I think I need you to start the ball rolling and give me some ideas about how Twitter might work for communicating with patients? Do you mean through private messages? Or do you think that if a patient asks you want their latest cholesterol is on twitter publicly, then it is OK to reply publicly because they have set the boundaries of the conversation? Before replying should we check "are you aware this is public?", or should we just reply anyway presuming that they know?

Even if the records could be tracked and audited and all the other issues you raise addressed, I would still not feel comfortable.
